Programme 6 of 7 · Article 5, Objective 6

Digital Care Coordination & Monitoring

Using practical digital tools to help care teams organise visits, follow up, and learn.

Our constitutional objective

Use simple technology for coordination and monitoring.

01

The problem

The Foundation’s proposal identifies limited community data and weak referral systems. As care involves more households and volunteers, registration, visits, changing needs, and follow-up information need to be organised and kept confidential.

02

The gap in support

A visit or referral has limited value if the next person supporting the household cannot follow up appropriately. The gap is a simple, secure way to coordinate care and report progress without excluding older people who have limited phone access or literacy.

03

What is being done

JAF’s proposal specifies digital registration, visit monitoring, referral tracking, and reporting. Possible tools include mobile data collection, a simple dashboard, and assisted reporting. A live JAF care-management system or completed digital rollout is not claimed on this website.

04

How JAF is closing the gap

JAF plans to develop or adapt affordable digital tools for registration, follow-up, referral tracking, monitoring, and reporting. Volunteers will assist with records while face-to-face care continues, so technology supports access rather than becoming a requirement for help.

Turning the response into action

JAF’s programme activities

  1. Develop or adapt a simple beneficiary register and visit-follow-up process.
  2. Track referrals and actions needing follow-up by the care team.
  3. Prepare monitoring reports to support coordination, learning, and accountability.
  4. Build informed consent, confidentiality, and secure storage into digital and paper records.

What this programme aims to change

Care teams can coordinate support more reliably while protecting personal information and maintaining face-to-face access.

How progress will be tracked

  • Completeness and quality of care records
  • Visits and referrals recorded for follow-up
  • Monitoring reports completed
  • Staff and volunteer feedback on tool usability

These are intended outcomes and proposed monitoring measures, not reported results.

A little care can mean
a great deal.

Help build a community where older people feel valued, included, and supported.
