Programme 2 of 7 · Article 5, Objective 2

Community-Based Care & Home Visits

Bringing care and companionship closer to older people, in the places they call home.

Our constitutional objective

Provide community-based care and home visits.

01

The problem

Older people living alone or with mobility challenges may struggle to reach community activities and services. JAF’s proposal describes households facing illness, disability, loneliness, and limited family support, where everyday needs can go unnoticed.

02

The gap in support

A single visit or occasional gift cannot provide the consistency of regular care. The gap is an organised system of trained visitors, repeat contact, and follow-up when health, wellbeing, or protection concerns arise.

03

What is being done

JAF has documented community visits, including a visit to Imat Aida’s home to deliver an Easter package. These activities demonstrate personal contact and practical support. A regular trained-volunteer visiting schedule is part of the proposed expansion.

04

How JAF is closing the gap

JAF aims to establish regular home visits by trained community volunteers and caregivers. Visits will combine companionship with wellbeing checks, awareness of hygiene and nutrition, recognition of concerns, and connections to appropriate services.

Turning the response into action

JAF’s programme activities

  1. Recruit and train community volunteers and caregivers for respectful, safe home visits.
  2. Arrange regular visits and follow up on needs identified during earlier contact.
  3. Offer companionship and listen to the older person’s own priorities and choices.
  4. Recognise health or protection concerns and connect people with qualified providers or appropriate local support.

What this programme aims to change

Older people receive more consistent personal contact, with concerns recognised and followed up earlier.

How progress will be tracked

  • Home visits completed
  • Older people receiving regular visits
  • Concerns followed up or referred
  • Beneficiary feedback on support received

These are intended outcomes and proposed monitoring measures, not reported results.

A little care can mean
a great deal.

Help build a community where older people feel valued, included, and supported.
